Student-Athletes Responsibilities in Academic Schedule Conflict
Student athletes will, on occasion, have varsity athletic contests scheduled at times which conflict with regularly scheduled academic classes. At the beginning of the semester, complete this form and have it signed by each of your professors where a conflict may occur.

Use this form in situations where you are confronted with obligations or responsibilities (ranging from participation in field trips in the visual arts or the sciences or rehearsals or performances in the performing arts to extra-curricula activities at which students are representing the college such as model senate events or varsity athletic contests) that conflict with regularly scheduled academic classes.

Print out this form when you need to add or drop a class after the commencement of the first day of class. Adding classes will require a signature from the class professor and your adviser. Dropping courses require only the adviser's signature. Please note: If you are not able to get hold of the teacher or adviser you may attach email permission from them to the add/drop form.

Print out this form and make an appointment to meet with the chair of department of your intended major. The chair will assign you an adviser in the major and will sign this form. Once complete, send the form to the office of the Associate Dean for Undergraduate Studies.

IIM must declare by the 8th week of spring semester, sophomore year.
Print out this form and make an appointment to meet with the chair of department for Interdisciplinary Studies. Once complete, send the form to the office of the Associate Dean for Undergraduate Studies.
Changes to Declaration of Major(s), Minor(s), and Concentration(s)
Print out this form and make an appointment to meet with the chair of department of your newly intended major. The chair will assign you an adviser in the major and will sign this form. Once complete, send the form to the office of the Associate Dean for Undergraduate Studies.
Foreign Language Agreement
This form is for use by students who are not enrolling in Goucher foreign language courses, either because they are pursuing a language at a college other than Goucher, or because they are a native speaker of a language other than English.

Incomplete Grade Form
Incompletes are given only for reasons beyond the student's control. In all instances, the student must initiate the application for an incomplete with the instructor, and except in the case of a last minute crisis or illness, a student must make application no later than the end of the twelfth week of the semester. Incompletes are intended to apply to cases where approximately three-quarters of the work of the semester can be completed by the end of the semester. If the work to be done involves more than one-fourth of the course, and the reasons for the student's failure to complete the work are legitimate, the instructor may recommend to the associate dean that the student be permitted to withdraw from the course. When the parties fail to agree, the case may be referred to the provost.
Official Leave of Absence (LOA) Form
A student in good academic and financial standing may request a leave for one or two semesters. A leave begins at the end of a regular semester. The student is expected to return at the conclusion of the leave or to request an extension of the leave.
Pass/No pass Form
A student may elect to take a course pass/no pass. By doing this the course will not be counted toward your G.P.A. You are able to take up to two courses per year pass/no pass. Note: you may not take any of your required courses for your major pass/no pass.
Petition Form
This petition for an exception to College Policy is to be completed by the student and his/her advisor before submission to the Associate Academic Dean for Undergraduate Studies.
Please use this form to build your schedule before meeting with your adviser. This is very helpful when planning your schedule so class times do not conflict.
